Here at Hogan & Graceffo Dental Services our dental professionals are experienced and knowledgeable in all aspects of cosmetic dentistry. Dental Insurance Policy

Hogan & Graceffo Dental Services and Cato Dental Insurances do not participate with insurance. We are happy to help you submit your claim to your insurance for reimbursement, but you’re ultimately responsible for all fees, whether or not you have dental insurance.

Cancellation Policy

We kindly ask that you give our office 48-hours notice in the event that you need to reschedule your appointment. This allows for other patients to be scheduled for that appointment. If you miss an appointment without contacting our office within the required time, this is considered a missed appointment. A fee of $50.00 will be charged to you; this fee cannot be billed to your insurance company and will be your direct responsibility. No future appointments can be scheduled nor can records be transferred without the payment of this fee.

Additionally, if a patient is more than 20 minutes late without prior notice for a scheduled appointment, we will consider this a missed appointment and the $50.00 cancellation fee will be charged. If you have any questions regarding this policy, please let our staff know and we will be glad to clarify any questions you have. We thank you for your patronage.

Your First Appointment

Your initial appointment usually will consist of an examination, personalized discussion about your oral health, and a review of your treatment options. Usually, a teeth cleaning or treatment can begin the same day. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation:

Any x-rays taken by a previous dentist, if applicable.
If your previous dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
A list of medications you are presently taking.
